NRC INTEGRATED INSFECTION REPORT NO. 50-443/97 05
Dear Mr. Feigenbaum:
From August 25 to September 10,1997, a team of inspectors led by Mr. W. Raymond
conducted a 40500 Team inspection at the Seabrook Station. The inspection was
conducted to evaluate the effectiveness of your processes for identifying, resolving, and
preventing issues that degrade the quality of plant operations and safety. The preliminary
findings were discussed svith you and other members of your staff at the conclusion of the
inspection on September 10,1997.

Baced upon the team's limited review, the overall implementation of your problem
identification and resolution process appears to vary in performance with some
improvement noted since our previous review. The team noted an evolving self-
assessment and corrective action process, that is now consolidated and with some
demonstrated successes in assuring safety end quality of operations. There were notable

initiatives in place or under development to further critical self evaluation and to improve
performance. Additionally, the front end of this process on problem identification appears
to be working well. However the team's findings include notable difficulties in effectively

addressing some performance issues such as the reduction of human errors and repetitive
.
discrepancies in the station tagging processes. Barriers to success in the back end of the
process on effective problem analysis and resolution included the need to more
consistently identify the causes for performance issues, and take effective corrective
measures based on trends, to effectively deal with the backlog of a high volume low
threshold process, and to effectively control process improvement changes. Except as
noted below, these problems have been recognized by you; the effectiveness of your
recent initiatives remains to be demonstrated.
Within the scope of this inspection, no violations were identified. One area of particular
concern was the adequacy of the station work control procedures for the deficiency
evaluouan process. The team found that the process as described in proc 3dures MA 3.0
and 3.1 had weak programmatic controls in that system engineer responsibilities are not
well defined and formalized with controls, and there was an unclear linkage to processes
that address safety evaluations and plant design changes. We understand that, in
