50-328 SEQUOYAll NUCLEAR PLANT (SQN) - ACTIONS TAKEN AS A RESULT OF DECEMBER 15, 1986 REQUALIFICATION EXAMINATIONS In compliance with a request made by J. Munro, of your staff in a telephone conference call conducted on January 31, 1987, please be advised that the Senior Reactor Operator (SRO) and the three Reactor Operators (R0s) who failed the requalification written examinations administered by NRC to SQN licensaes on December 15, 1986, were removed from licensed duties effective February 2, 1987.

The licensees removed were J. M. Alexander, License Number SOP-3630-4, Docket Number 55-6701; C. E. Dyer, License Number OP-20320, Docket Number 55-7931; E. L. Hyden, License Number OP-20256-1, Docket Number 55-20515; and S. R. Johnson, License Number OP-20257-1, Docket Number 55-20207.

Mr. Alexander has been removed from shift and reassigned to a position in maintenance which does not require a license.

A letter will be forwarded to NRC requesting that his license be placed in inactive status.

The R0s have been assigned temporary duties which al w do not require a license.

These individuals will not be permitted to perform licensed duties until they have been retrained and pass a requalification examination.

The Division of Nuclear Training (DNT) is now developing a program to retrain and requalify these licensees.

The program is tentatively scheduled to begin in late February 1987.

We will inform NRC of the actual dates when they are confirmed.

In compliance with Mr. Munro's request, TVA will forward the final examination and answer key for your review before administration of the examination.

DNT has analyzed the written examinations for weaknesses.

No weaknesses were identified among the SR0s.

The following weaknesses were identified among the R0s examined.

Emergency Operating Instructions - bases, procedure steps, and actions to be taken when desired response is not obtained 2.

Emergency boration Instruction in these topics will be administered in the retraining program.

The areas of knowledge reflected by the examination questions are being addressed in week one of the 1987 Requalfication Program for all Sequoyah licensees. The weaknesses identified by TVA will be addressed in requalification training _for all R0s and SRO licensees during this year as required by 10 CFR 55, Appendix A.
