Statement of Violation:

' Technical Specification 6.8.1.a requires, in part, that written precedures be established,

implemented, and maintained conceming the applicable procedures recommended in

Appendix A of Regulatory Guide 1.33, Revision 2, Febtaary 1978. Regulatory Guide 1.33, Appendix A, Section 9.c.(l), states that procedures should include information on

the method for obtaining permission and clearance for work. This requirement is

implemented, in part, by Plant General Procedure OPGP03-ZO-EC01, Re.ision 6,

" Equipment Clearance Orders." This procedure requires that equipment clearance orders

shall provide adequate personnel and equipment safety.

Contrary to the above:

On June 24, Equipment Clearance Order 97-2-76518 failed to provide adequate personnel

and equipment safety in that Essential Cooling Water Screen Wash Booster Pump 2A

inadvenently started while the system was drained.

Also, on July 21, Equipment Clearance Order 97-1-71609 failed to provide adequate

personnel and equipment safety in that workers breached an unisolated component -

cooling water system pipe while performing maintenance on Residual Heat Removal

Pump 1B.-

This is a Severity Level IV violation (Supplement 1)(498;499/97005-03).

II.

South Texas Project Position:

South Texas Project concurs that the violation occurred.

III.'

Reason for the Violaticn:

The cause of the June 24, Unit 2 Screen Wash Booster Pump event was the failure of the

. Senior Reactor Operator to identify the additional relay contacts that actuate the Screen Wash

Booster Pump during his review of the Equipment Clearance Order for the Traveling Screen.

. The July 21, Unit 1 Residual Heat Removal event was caused by less than adequate

.

communications between the work package planner and the Equipment Clearance Order

preparer. In addition, the Equipment Clearance Order acceptor did not perform an adequate

. walkdown verification.

IV.

COltitECTIVE ACTIONS:

As a result of the Scicen Wash Ilocster Pump event, the following cornctive actions have been

initiated.

1.

The 2A Screen Wash llooster Pump was tested and verified to be operable.

2,

The Work instructions for the implementation of this DCP on Unit I were revised

to include a precaution that identifies this potential event.

3.

'lhe individuals involved in this event were counseled on the importance of

correct Equipment Clearance Orders.

As a result of the flesidual llcat Removal Pump issue, the following actions have been initiated

was reinforced to all Equipment Clearance Order preparers and reviewers during

Management's expectations for Equipment Clearance Order acceptance were

reinforced to Maintenance personnel during special meetings. These activities

were completed September 10,1997.

These shon tenn corrective actions will resolve the issues identified in this Notice of Violation.

The actions described in section VI are designed to ensure all other issues that may exist . ire

identified and permanently corrected.

VI.

Additionallnformation

Ilased on this violation and other related issues that we have previously identified

through our corrective action program, the following actions are also planned.

(

A Project Manager has been assigned to lead a multi discipline team in a

comprehensive process review of the STP Nuclear Operating Company

liquipment Clearance Order procedure ( including the use of danger tags, caution

tags and test tags) and the plant configuration progr..m.

2.

The general scope of this review will be to identify industry best practices,

perform a comparison with the STP Nuclear Operating Company program,

j

identify areas for improvement, modify the procedure, and train station personnel.

These actions will be completed by the end of the second quarter of 1998.
