Gentlemen:

' Per my telephone conversation with Mr. Tom Headly, NAC International, the DOE /NRC Form 741 Nuclear Material Transaction Report submitted on September 12,1997 contained three errors which need correction. ne first was that the receiver code in line 7 needs to be deleted and

- the block left blank. De second was that the lines 24B and 25B need also to be left blank, thus the "SD" needs to be eliminated. And lastly, the material type in lines 24F and 25F needs to be changed from 33 to 20. All of these corTections have been mxle in a pen and ink fashion on the attached DOE /NRC From 741. Please update your files appropriately.
