Tsco:3 University of Utah Docket No.: 50-072 Salt Lake City, Utah 50-407 License No.: R-25 R-126 EA 86-136 During an NRC inspection conducted on June 9-12, 1986, violations of NRC requirements were identified. The violations involved the failure to implement and maintain the Physical Security Plan (PSP). In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1986), the violations are listed below:

1. Chapter 1, Section 1.1 of the PSP, states, in part, that steel yy doors 4 and 5 provide access to the radiochemistry laboratory from the reactor room. When authorized personnel are not o[/phe present within the University Nuclear Engineering Laboratory, 'y doors 4 and 5 are secured by dead bolt locks. 3 Contrary to the above, from June 9-11, 1986, door 4 was not g locked when the University Nuclear Engineering Laboratory was g unoccupied. 2. Chapter 1, Section 1.1 of the PSP states in part that the over-head crane is shut off at a lockable switch box within the con- O q trolled access area (Room 1001-E). During periods when the crane D is not in use, the power at the switch box is turned off and the $' Q o switch locked. D Contrary to the above, from June 9-11, 1986, the overhead crane M switch was not locked when the crane was not in use. b

trolled by the Mechanical and Industrial Engineering Office, e ] g physical plant, and the university key shop. These keys are only f, issued to staff of the university and no students are issued m N y

these keys. Keys to door 3 (AGN-201 and TRIGA Reactor Room 1001-E) \j are maintained by the reactor supervisor and only three keys are maintained. One key is retained by the reactor supervisor, jgoS8MI8588lb G

Contrary to the above, on June 11, 1986, a student possessed a key to door 2 and 3, and the third key to the TRIGA and AGN-201 reactor room (Room 1001-E) was not being kept in a locked box, but was kept in an unsecured electrical fuse panel in a file cabinet in Room 1001-C.

These violations have been categorized in the aggregate as a Severity Level IV problem (Supplement III).

B. Failure to Maintain Facilities and Equipment v1 :c

1. Chapter 1, Section 1.1, of the PSP states, in part, that R @

Contrary to the above, as of June 9-11, 1986, 100 percent of the @

view afforded by the window in room 1001-A was obscured by a S stack of empty soda pop cans placed on the window sill area, and l

approximately 50 percent of the view from room 1001 windows was  % [@;!

obscured by a chart, the AGN-201 reactor console, and books 1C which were stacked on the AGN-201 console.

2. Chapter 2, Section 2.1 of the PSP states that violation of the g

intrusion alarm system results in the transmission of a signal g interrupt over a dedicated telephone line to the University of s 5 Utah Police Dispatcher and to the University Radiation Safety R Office (Radiological Health Department). In each of the offices @

Contrary to the above, as of June 10, 1986, the alarm at the Ra- e Q diological Health Department had been disconnected. ,,_ o These violations have been categorized in the aggregate as a Severity 'I Level IV violation (Supplement III). h N

C. Failure to Conduct Audits N W

The AGN-201 Technical Specification 6.4.3, states, in part, that audits of facility activities shall be performed under the eg kg cognizance of the Reactor Safety Committee. These audits shall be $g conducted at least every two years and shall examine the operating

  • 00 records and encompass the Facility Security Plan and implementing Q procedures.

Contrary to the above, since May 2, 1979, audits of the Facility Security Plan had not been conducted.

This is a Severity Level IV violation (Supplement III).

a s Appendix Pursuant to the provisions of 10 CFR 2.201, the University of Utah is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
