description of investigations conducted to determine the cause of Calvert Cliffs Unit 1 Steam Generator tube degradation seen during the recent October 1986 Inservice Inspection and provide information about corrective measures l

Dr. Thomas E. Murley February 12, 1987 Page 2 The initial eddy current testing plan for the October / November 1986 outage called for approximately 16% of the tubes in Steam Generators 11 and 12 to be inspected.

From this inspection plan 3% of the tubes in each Steam Generator were pre-selected as the Technical Specification population to which the inspection results classification rules would be applied.

Following analysis of the data from the Technical Specification sample the results were determined to be in Category C-3 and the requirement to expand the inspection to 100% was carried out for both Steam Generators.

l Thirty-two tubes were found to be defective, 21 in Steam Generator #11, and 11 in Steam Generator #12.

These tubes exceeded the plugging limit of 40% loss of nominal wall thickness.

Additionally, six tubes which showed eddy current indications of less than 40% nominal wall loss were plugged.

1 Four of these tubes were conservatively plugged due to degradation at supports, one tube was plugged due to an over expanded region above the tube sheet and one tube was plugged because a section containing <20% indications was removed for-metallurgical examination.

Enclosures 2 and 3 list each tube plugged and the reason for plugging. identifies the support location nomenclature used to identify NDE results relative to these tube locations.

The table on the following page is a summary of the 1986 Unit 1 inspection results.

probes, narrow field bobbin coil
probes, permanent magnetic suppression bobbin coil probes, and profilometry probes.

With the exception of profilometry these inspection methods were used to further quantify indications.

Profilometry was used to evaluate tube denting at the 9th and loth solid support plates.

contain various plots showing Steam Generator #11 and #12 eddy current indication locations.

The tube addy current indications can be placed into one of three major location categories:

o just above the top of the tube sheet, o

at supports, or in free span areas between and unassociated with supports'.

o It is apparent that the indications found just above the top of the tube sheet exist primarily in regions of higher sludge levels above the Steam Generator primary inlet side.

Those indications in this region above the outlet side also correspond primarily with higher sludge levels.

Most of these-indications produce relatively low magnitude signals.

Motorized rotating pancake coil data indicates that these indications are localized with respect to the circumference and do not have significant length.

Their geometry appears to be pit-like in nature.

Tube specimens containing these type of indications were removed from Steam Generator #11 and are undergoing metallurgical and chemical evaluations.

Eddy Current indications seen at support locations appear to be caused by tube wear.

One tube specimen containing an indication at a support was removed in November 1983.

Wear marks at support contact points were confirmed.

Note that most indications at supports occur at the vertical middle (VM) support and to a lesser degree at the diagonal (DH & DC) and vertical (VH & VC) hot and cold leg side supports.

Also note that there seems to be a higher occurrence of vertical middle support location indications between rows 9 and 11 inclusive.

Eddy Current indications seen in free span areas are typically low magnitude signals.

Specimens containing free span indications that were <20% were removed from one tube in November 1983.

Murley February 12, 1987 Page 5 removed in 1983 were somewhat inconclusive and suggest that the indications were manufacturing imperfections, caused by false signals from deposits on the tube and/or shallow crystallographic wastage attack which may have occurred preservice or inservice.

Specimens from two tubes containing four free span indications were removed this outage for additional study.

It was noted that there seems to be no correlation of free span indications with respect to location, elevation, inlet side or outlet side.

Altogether portions of six tubes were removed from the inlet side of Steam Generator #11 during this inspection.

The tubes were selected from the 1986 eddy current inspection results with emphasis on the sludge pile region of the steam generator.

These tube samples are undergoing additional testing including metallurgical and chemical evaluations.

Corrective measures have been initiated to minimize further degradation to the Steam Generators.

During the 1986 Unit No.

1 outage we completed the removal of all major copper components from our feedwater and condensate systems.

We are completing installation of a nitrogen blanketing system on all condensate storage tanks to reduce auxiliary feedwater and condensate makeup oxygen.

We are evaluating use of morphline a

for minimizing the ingress of solids from the feedwater system into the Steam Generators and possible sludge pile cleaning or removal.

Modification of our current condensate precoat filters is scheduled for 1987-1988.

This in conjunction with our full flow deep bed condensate polishers will provide additional protection of our steam generators from impurity ingress from our condensers.

Our current chemistry procedures require notification and action to be taken below the SGOG PWR Secondary Water Chemistry Guideline Action Levels.

This enables us to take action before conditions become degraded in the Steam Generator.

During outages Steam Generators are kept in wet lay-up with a nitrogen blanket as much as possible.
