MODIFICATION NO. 4 TO TASK ORDER NO. 3 ENTITLED " NATIONAL SEALED SOURCE AND DEVICE REGISTRY SYSTEM" UNDER CONTRACT NO. NRC-02-95-001
Dear Ms. Parker:
The purpose of this modification is to: (1) revise the estimated completion / deliverable schedule reflected under Section 8.2 of this task order and (2) provide for an expanded deliverable, as consideration for having to revise this schedule. Consideration is required because i correspondence from Jupiter stated that part of the delay in not submitting the deliverables by the established due date was associated with the transition from one PowerBuilder Programmer to another. This does not represent an excusable delay as defined by FAR 52.249-14. The consideration is an electronic text version of the final versions of the deliverable documents listed under subsection 8.1 of the Statement of Work for this task order. This task order modification incorporates the expanded deliverable under subsection 8.1. Accordingly, the task order is hereby modified as follows:
Under Section 8.2 entitled " Specific Scope," the completion / delivery dates for certain work to be delivered for front end application development and data conversion are revised to read as follows:

- Development of Data Entry Module 5/30/99
- Development of Module for Integration 7/07/99 with ADAMS
- Draft Front End Application 5/30/99" Data Conversion
- Perform Data Conversion 5/30/99" Under Section 8.0, " Deliverables / Schedule," of the Statement of Work to the subject task order, subsection 8.1, after the third paragraph, ADD the following paragraph as consideration for the J; l schedule adjustment:

All other terms and conditions of Task Order No. 3 remain unchanged.
