As I explained in my telephone call of May 27, 1992, we require additional information about the incident you reported in your letter dated February 26, 1992.
Please submit the following information about the incident, as required in section 34.30(b),
10 CFR Part 34 so that we can complete our files.
. Manufacturer and model number of the equipment involved in the incident (34.30(b) (3)) - specifically, tTe model number and serial number of the sealed source used in the incident.
. Place, time and date of the incident (34.30(b (4)).
. Actions taken to establish normal operations (34.30(b) (5)) -
what you and the radiographers did to discover what happened and then to eliminate the hazard.
. Qualifications of personnel involved in the incident (34.30 (b) (7) ) - hours of training, special training received, years of experience as radiographers or assistants, years with the company.
