ENCLOSURE 1 NOTICE OF VIOLATION Westinghouse Electric Corporation Docket No. 70-1151 Columbia Nuclear Fuel Plant License No. SNM-1107 During the Nuclear Regulatory Commission (NRC) inspection conducted on January 26-30, 1987, a violation of NRC requirements was identified. The violation involved failure to maintain and follow approved radiation protection procedures. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1986), the violation is listed below:

License No. SNM-1107, Condition 9 required that licensed materials be used in accordance with statements, representations, and conditions contained in Chapters 2, 3, and 4 of the Application dated January 4, 1985, and supplements thereto.

Paragraph 3.2.1.1 of the Application states that written procedures describipg general radiation protection requirements shall be maintained and followed.

Regulatory Affairs Procedure RA-200, " Protective Clothing," Revision 6, December 16, 1986, Paragraph 2.3, required that when personnel exit the Controlled Area, they are to wash their hands thoroughly and monitor per posted instructions.

Regulatory Affairs Procedure RA-203, "Ceneral Health Physics Rules and Recommendations," Revision 7, December 7, 1986, stated that posted instructions at contamination control step-off-pads and change rooms shall be followed.

Regulatory Affairs Procedure RA-217, " Personnel Monitoring," Revision 0, December 18, 1986, Paragraph 2.0, required that when personnel exit the Controlled Area, they are to follow posted instructions for monitoring (frisking).

Contrary to the above, the requirement that procedures be maintained and followed was not met in that, on January 29, 1987, individuals exiting the controlled area at the main step-off pad were observed by inspectors not to follow the posted instructions for frisking as required by Regulatory Affairs Procedure RA-200, RA-203, and RA-217. Individuals held the instrument probe at distances greater than the specified distance (contact) from their bodies; moved the probe in rapid, sweeping motions, such that the instrument would not detect the contamination limit; failed to wash their hands prior to frisking; failed to perform a survey of the bottom of the soles of their shoes; and one individual failed to frisk at all.

Pursuant to the provisions ~ of 10 CFR 2.201, Westinghouse is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including: (1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4)the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
