This refers to the Nuclear Regulatory Commission (NRC) Operational Safety Assessment conducted by T. D. Lee,and other members of the team on January 12-16, 1987.
The inspection included a ' review of activities authorized for your Columbia facility.
At the conclusion of the inspection, the findi,ngs were discussed with those members of your staff identified in the enclosed inspection report.
,s Areas examined during the inspection are identified in the report. "dithin these areas, the inspection consisted of selective examinations of procedoras and representative records, interviews with personnel, and observations 4f activities in progress.

Your attention is invited to an unresolved item identified in paragraph 6.b 2 cf the inspection report. This matter will be pursued during future inspectionst The findings of this inspection indicate that weaknesses exist in some aspsets of your operational safety program and that there are program areas which sh_ould be, evaluated and considered for improvement.
These are discussed in Appendices A' and B which are titled " Operational Safety Assessment Weaknesses" and j
" Operational Safety Assessment Improvement Items" respectively.
We recognize that explicit regulatory requirepents pertaining to each item identified in Appendices A and B may not currently exist _ Notwithstanding this, you are requested to submit to this office the following information within 90 days of receipt of this report:

the actions you have taken or' plan to take en the weaknesses specifird in Appendix A, and

2.
the results of your consideration of each of the improvement ttems specified in Appendix B.
