Documentation used to support this Quality Assurance program includes the Radioactive Material Transport Data Sheet and the Source Data Record Summary (see enclosures 1 & 2).
'Ihe training of personnel is acccrnplished through Nova Data Testing labs Radiation Safety program. No person shall handle, ship, or receive material without first completing the necessary 8 hour9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> Radiation Safety Course.
All packaging material used shall be manufactured under the control of an NRC-approved Q.A. program. A certificate of canpliance will certify the packaging devices meet all DOT requirements.
The operation and maintenance procedures recmmended by Gamna Industries for the Century SA and C-10 source changer shall beused as guidelines for their operation.
The following steps will be taken to ensure the safe handling and shipping of radioactive exposure devices.
SIIIPMENTS: Packaging and Opening Containers Receipt of Scaled Sources Scaled sources received from authorized shippers (e.g. anery) shall be handled by gaalified radiographic personnel.

Prior to opening source container, personnel shall be provided with their assigned film badges, pocket dosimeters, and an operating, calibrated survey meter which has Men checked for operation.
2.
Source container shall be surveyed at the container surface, and record on receiving receipt. 'Ihe limit of radiation levels shall be no more than 50 mR/hr.
If this limit is exceeded the source con-tainer shall be removed to an isolated area under surveillance and the Radiation Safety Officer or his delegated assistant shall be called 4
to supervise further steps.

A source container with surface radiation levels less than the limit imposed by (2) above may be unsealed and opened. An inmediate physical check shall be made to insure that locks and seals are in place.

A source projector with radiation levels within the limits set by (3) above shall then be placed in the cmpany's source vault and secured.
SHIPMENTS: Sealed Source Packaging for Shipnent Scaled sources to be packaged for shipnent shall be handled by qualified radiographic personnel under the supervision of a Radiographer.

Prior to rmoving sealed source frm vault, personnel shall be provided with their assigned film badges, pocket dosimeters and an operating calibrated survey meter which has been checked for operations.
2.
Scaled source shall be physically inspected inmediately on removal frm vault; locks and seals shall be in place. Radiation levels at 6" frm all exterior surfaces shall not exceed those specified above.
3.
If seal is not in place on the projector locking mechanism, a seal shall be affixed after it has been determined that the source is in the fully housed position.
4.
After placing the projector and control cables in the source container, a packing slip shall be empleted indicating:
a.
The source material, activity and serial number.
b.
W e projector model and serial number.
c.
The name, address and telephone number of the shipper.
d.
The name, address and telephone number of the consignee.
The disposition to be made by the consignee.
e.
f.
A cmpleted copy of a Radimetive Material Transpcrt Data Sheet.

'Ihe source container lid shall be attached and secured with a bolt and nut fixed in such a manner that a seal may be affixed which will prevent removal until delivery to the consignee.
6.
A packing slip and Radioactive Material Transport Data Sheet shall be attached to the surface of the container indicating the information required above.
7.
A radiation survey must be performed at the surface of the package and at one meter from the survey to determine the proper shipping labels to be applied to the package. Use the criteria in Table 1.
8.
Properly cmplete two shipping labels indicating the contents (i.e.:
Iridium-192); the number of curies and the transport index.
9.
Assure that any old shipping labels have been removed frm the package.
Apply two properly completed shipping labels to two opposite sides of the package. Delivery of a sealed source, in a container, to the-shipper must be accmpanied by an operating, calibrated survey instru-ment. Shipping documents shall be delivered inmediately to the receiving clerk to be addressed to the Radiation Safety Officer for log entries.
Sealed Source Packaging for Interstate or Intrastate Transport (On Job Requirements)
Follow steps required for Sealed Source Packaging for Shipnent.
The Radiographer shall emplete the appropriate documents and have the documents in his possession. Source container shall be secured in the transport vehicle by adequate blocking, bracir or tie-down of the source container to prevent its shifting or movemen; s ring normal transport.
Vehicle shall be placarded on all four sides if the package ~(source container)-
required a Yellow III' label.

SOURCE CHANGING Source Changing and Receipt and Disposal of Sources All source exchanges and shipnents will be conducted under the personal supervision of a Lead Radiographer for Radiation Safety Officer.
Only the Gama Industries Model C-10 Iridium-192 shipping container will be utilized.
W e step-by-step instructions issued by the manufacturers will be followed in detail and in the sequence presented. Particular care must be taken to assure the source is securely locked in the fully shielded position as confirmed by carefully performed radiation surveys.

All source shipnents will be made in accordance with current D-O-T Regu-lations and this program.
All incoming sources must be surveyed within 3 hours3.472222e-5 days <br />8.333333e-4 hours <br />4.960317e-6 weeks <br />1.1415e-6 months <br /> of receipt and the results of_ surveys documented. Any incming sources found to have radiation levels in excess of D-O-T regulations will-be imnediately renoved to a safe excluded area and the RSO notified.
All pertinent Q.A. records shall be kept in the Radiation Safety File.
