We have reviewed your letter dated January 14, 1986 requesting amendments to License Numbers 21-03280-01 and 21-03280-02 and find that we will need additional information as follows:
- 1. Your source accountability program should be strengthened considerably.
In order to accomplish this, you should either provide the following or else develop and submit equivalent alternatives:
- a. Designate one individual as source custodian and develop a procedure to ensure that all sources are ordered, received, stored, dispensed and returned to storage after treatment through this individual,
- b. Develop and submit specific procedures to ensure compliance with 10 CFR 35.14(b)(5)(vii), enclosed.
- c. Develop a schedule for periodic inventories (e.g., monthly) to compare the actual source count to the running inventory.
- 2. Please confirm that linens will be checked with a survey meter prior to removal from the patients room.
- 3. Our files do not indicate that Dr. Mohammad Z. Iqbal is currently listed on either of the Mount Sinai Medical Center licenses (License Numbers 48-03280-01 or 48-03280-02). Please submit a description of Dr. Iqbal's training and experience on the enclosed preceptor forms or copies of an NRC or Agreement State License which authorizes Dr. Iqbal to perform sealed source therapy.
