In accordance with 10CFR55.23 and 10CFR55.33, Southern Nuclear Operating Company (SNC) is submitting a revised Certification of Medical Examination form (NRC Form 396) for License #0P-20745-1/ Docket #55 21259. The licensee has undergone a surgical procedure (radial keratotomy) to correct refractive error. A recent eye examination indicates that his uncorrected vision now meets NRC minimal requirements for both far and near vision.
