Thank you for your letter of May 28, 1986, in response to our letter dated April 28, 1986. As a result of our review, we find that additional information, as discussed with your Mr. M. Berberian during a telephone call on June 6,1985, is needed. Specifically:
Appendix A, item A:
The stated Foxboro actions to prevent recurrence are existing programs which were found to be ineffective by the inspectors during the NRC inspection.
However, the NRC Office of Inspection and Enforcement (IE) currently has a pending information Notice (IN) which discusses the insulation degradation problem. The IE/IN will provide notification to all nuclear plants.
Therefore., please submit (1) a description of any new or additional steps that will be taken to prevent recurrence; and (2) the dates your preventive measures will be completed.
Appendix A, item B:
No description of steps were provided for correction of the identified inade-quate 10 CFR Part 21 procedure.
Completion dates were also not provided.
Additionally, Foxboro has not specifically stated whether or not 10 CFR Part 21 reportability evaluations were performed for the six examples of defects which were found during the NRC inspection.
For items c, d, and e, under Section B of Foxboro's response, the logic given for not reporting under Part 21 is "no failure history" and "not a common mode or generic failure." Part 21 does not provide any relief from reporting requirements on those bases. Any deviation must be evaluated or reported to all affected customers and all defects must be reported to the NRC.
Therefore, please submit for violation B (85-01-02):

Appendix B, item A:
No description of steps to prevent recurrence were provided. The dates of completion were not provided in the response.

(1) a description of steps that have been or will be taken to prevent recurrence; (2) the dates your preventive measures were or will be completed; and (3) the date that TFC has or will issue the 10 CFR Part 21 directive, as discussed in TFC response letter.
Appendix B, item B.1:
No completion dates were provided.
Therefore, please submit for nonconformance B.1, (85-01-04):

Appendix B, item B.2:
The reference to Appendix A, Section B is correct in part. The referenced Section B discusses corrective actions but fails to specify actions to pre-clude recurrence and the completion dates.
Therefore, please submit for nonconformance B.2 (85-01-04):
(1) a complete description of steps that have been or will be taken to correct this item; (2) a description of steps that have been or will be taken to prevent recurrence; and (3) the dates your actions were or will be completed.
All of the other portions of your reply have been found to be responsive to the concerns raised in our Notice of Violation and Nonconformance. We will review the implementation of your corrective actions for the following listed items during a future inspection to determine that full compliance has been achieved and will be maintained.
