Apparent exposure in excess of the quarterly limit for hands Gentlemen:

This letter is to comply with the provision in 10CFR20.405(a)(1)(i) requiring reporting of exposures in excess of limits in 10CFR20.101(a).

On July 29, 1986 we received a call from our dosimeter vendor, i

R.S. Landauer Jr. & Co., that a TLD finger dosimeter for the month of June 1986 had recorded 23,560 mrem.

The dosimeter had been worn by an employee June 9, 1986 while packaging Tm-170 wafers for shipment.

Following the phone call, a committee to investigate the exposure was activated as per our Standard Operating Procedure HP-22, Review of Unplanned, Unusual Radiation Exposures.

The employee's work on June 9 was closely monitored by Health Physics personnel, as it had been for two previous, similar operations performed by the employee, for which the highest recorded hand exposure was 530 mrem. The investigation is still active.

In the meantime, we are showing the 23,560 mrem in the employee's exposure record.

If the final conclusion of the investigation indicates that the recorded value should be changed, this will be changed only af ter recommendation by the HP-22 Committee.
