50-424/86-07, 50-425/86-03 Attention: Mr. Luis A. Reyes The Georgia Power Company wishes to submit the following information concerning the violation identified in your inspection report 50-424/86-07 and 50-425/86-03:
Violation 50-424/86-07-02, " Failure to Protect Permanent Plant Equipment"
- Severity Level IV The violation indicates that maintenance activities were not accom-plished in accordance with applicable procedures in that the internal heater for motor-operated valve (MOV) 1-HV-11600 was not energized and the motor internals were at ambient temperature.
Georgia Power Company offers the following response pursuant to the criteria of 10 CFR 2.201:
Georgia Power Company acknowledges that the internal heater for M0V 1-HV-11600 was not energized.
As indicated in the USNRC inspection report, preventive maintenance was performed on M0V l-HV-11600 on February 6,1986, in accordance with the preoperational maintenance form attached to Maintenance Work Order (MW0) 18600845.
This preventive maintenance verified that the internal heaters were energized through temporary power cables as required.
The next preventive maintenance was not scheduled until May 18, 1986.
On February 13, 1986, the USNRC inspector observed that the temporary power cable had been cut and removed and the motor internals were at ambient temperature.
The USNRC inspector's observation was the first indication that the internal heaters had been de-energized.
On that same day, Operations Deficiency Report (0DR) T-1-86-493 was issued and predispositioned in accordance with Startup Manual procedure SUM-23.
Also on February 13, 1986, Maintenance Work Order 18602222 was issued to reconnect temporary power to the internal heater of M0V 1-HV-11600 and to megger test the motor windings.
The maintenance work order was completed on that same day and the megger test results were satisfactory.
These actions were performed in - accordance with the commitments made in response to Violation 50-424/85-40-04.

l:4 Page Two Procedural controls were in place to identify, disposition, and correct discrepancies cited in the Notice of Violation.
If the USNRC inspector had not discovered the de-energized heater, it would have
.been found during the next preventive maintenance and dispositioned in the same manner as described.
Additional documentation exists (other ODR's and MW0's) that confirms that this program effectively identifies-and dispositions this type of problem, and that the Georgia Power Company will ensure no damage affecting equipment operability has occurred prior to placing equipment into service.
In addition to the existing programmatic controls, further design information indicates that the M0V space heaters may not be necessary to maintain equipment qualification.
An open design change package-(CCP #B10291E, Revision 0) removes the subject space heater (as well as some others) from use.
The technical basis for this change was the conclusion that the heaters presented a generic condition that could cause damage (burning) to wiring insulation within the motor operator.
However, in order. to reduce the possibility of power interruption to i
remaining M0V space :. eaters, the Georgia Power Company has accelerated the program that will replace all turned over temporary power supplies with permanent power supplies.
Affected heater circuits are expected to be powered by permanent power supplies by September 15, 1986.
