Dear Mr. Ruger:

We were informed by Comonwealth Edison Company officials on April 1,1986 that you deliberately made an entry into a record associated with the operation of the LaSalle County Nuclear Station, Unit 2 without an adequate basis for knowing that entry was true.

Upon discovery of the erroneous record entry, Commonwealth Edison recognized the potential safety significance of the matter and investigated the incident. On two occasions, specifically, March 19 and March 24, 1986, you were asked by Comonwealth Edison Company officials whether or not you made the erroneous entry. You denied making the entry on both occasions.

You finally admitted making the entry on March 31, 1986.

On May 9,1986, I met informally with you to seek an explanation for your actions and to discuss the seriousness with which NRC views this matter.

The Comission must have high confidence in both the technical competence and the personal integrity of reactor operators such as yourself who are entrusted with the substantial responsibility of operating the controls of a nuclear power reactor. Your actions demonstrated a significant failure to carry out your responsibilities and to appreciate the importance the Comission attaches to truthfulness and trustworthiness of reactor operators.

In particular, your initial action caused an incorrect safety record to be created at the LaSalle facility which, in turn, caused a violation of facility procedures and facility Technical Specifications calling for the adherence to procedures. When Comonwealth Edison investigated the matter, your failure to acknowledge your conduct thwarted its efforts to determine what had occurred.

It takes little imagination to see how quickly such concealment could have serious implications should there have been a need to resolve matters associated with an actual accident or personnel exposure.

I consider it necessary to fonnally express NRC's dissatisfaction with your performance on this matter.

You should know that serious consideration was given to suspension of your license for an appropriate term as a sanction for your unacceptable performance. However, Comonwealth Edison took disciplinary action against you. Based on your Company's actions and my..;eeting with you, I am limiting my actions against you to this letter of reprim:nd.
