ATTACHMENT 1.

Summary A review of control circuit logics for various BVPS-2 safety-related HV AC supply and exhaust fans has revealed several problens.

Specific-ally, following a loss of offsite power event (LOOP) and the subse-quent transfer to energency power, certain HVAC systems may be restar-ted and sequenced in a manner that is not in accordance with design requirements for those systens.

2.

Immediate Action Taken Analysis and specific corrective action was cannenced resulting in Advanced Change Notice (ACN) C-174.

Mr. L. M. Rabenau, Canpliance Engineer, Beaver Valley Unit 2, notified Mr. Lowell Tripp of NRC Region I of this concern on January 16, 1987.

3.

Description of Deficiency Five specific problems were identified:

a.

Certain safety-related HVAC systens are designed to operate with only one train of the systen running at one time.

However, in some cases, after a LOOP, both trains of an HVAC systen may start up autanatically.

This problen is applicable to systens located in these areas:

Emergency Switchgear Area (HVZ), Control Roan Area (HVC), and Main Steam Valve House Vault Area (HVR).

b.

The control logics for certain HVAC systens function such that if one train on the systen is operating, and a LOOP occurs, the other train will be automatically restarted once energency power is restored.

However, should the train being restarted incur a fail-ure, the other train will not autanatically start up.

This prob-lem is applicable to systens in the following areas:

Cable Vault and Rod Control Area (HVR), and Control Building Area (HVC).

c.

For the Control Room Area HVAC system (HVC), the energency supply f ans are autanatically operated after a sixty minute time delay which is initiated by a CIB/ radiation signal.

If a LOOP occurs after the timer has been energized, the timer resets and begins timing sixty minutes again once energency power is restored.

d.

The Emergency Switchgear Ventilation Systen (HVZ) is designed to maintain a constant fan discharge tenperature by the modulation of danpers for outdoor air, return air, and exhaust air.

In order to maintain temperature, the outdoor air and exhaust air danpers need to move in one direction (open/close) while the return air danper must move in an opposite direction (close/open).

Since systen requirenents cannot tolerate a fail-open return danper which would result in overheating of the energency switchgear area, all dan-pers were purchased fail-closed.

This results in allowing the operation of the subject three danpers in the sane direction.

e.

The Control Room Air Conditioning System (HVC) contains two refri-geration units which have a reset function that requires a manual reset at the equipment location after a LOOP event.

However, systen design requirenents necessitate an immediate restart following a LOOP and restoration of energency power.

4.

Analysis of Safety Implication a.

The present control circuits will autanatically start both orange and purple trains following a LOOP.

The affected areas are not designed to accommodate the increased air flow resulting from parallel operation of both trains.

b.

The HVAC train operating at the time the LOOP occurs is locked-out by the systen logic fran restarting once energency power is restored.

Thus, failure of the opposite train (which is a cred-ible single failure) could result in neither of the HVAC trains operating even though energency power is restored, leading to a loss of ventilation in the affected areas.

c.

The starting of the supply fans could be delayed beyond the sixty minute design requirement, and may occur after the depletion of the bottled compressed air systen.

This could result in no HVAC flow to the control room area.

a.

The current operation of the subject danpers renders then unable to maintain discharge air tenperature which may cause overheating in the energency switchgear area, e.

Because the refrigeration units do not reset automatically follow-ing a LOOP, a lack of coolant to this systen results.

5.

Corrective Action to Resolve the Deficiency ACN C-174 h as been superseded by ACN-D-0015, which describes the detailed corrective action (attached).

It generally calls for rewir-ing of sane control circuits and/or addition / deletion of timers in order for the systens to perform in accordance with design require-ments.

6.
