DUQUESNE LIGHT COMPANY Beaver Valley Power Station, Unit No. 1 Reply to Notice of Violation Inspection 86-18 Letter dated September 9, 1986 VIOLATION A (Severity Level IV; Supplement 1)
Description of violation (86-18-03)
Technical Specification 6.12.2, High Radiation Area, requires that entry into high radiation areas be controlled with locked doors to prevent unauthorized access and that the keys be maintained under the administrative controls of the shift supervisor on duty and/or facility health physics supervisor.
Operating Manual Chapter 1.48.5.C, Padlock / Locking Devices Administrative Requirement, requires the shift supervisor to control entry into the NSS key cabinet.
Contrary to the above, on July 29, 1986, three individuals obtained access to the unlocked NSS key cabinet containing keys to the high radiation area.
Entries into the NSS key cabinet were not controlled in that these individuals removed keys while the shift supervisor was not in the office containing the key cabinet.
Corrective Action Taken 1.
The NSS key cabinet was immediately locked and the combination dial spun.
This action has been completed.
2.
All Operations personnel were notified that the NSS key cabinet shall remain locked with entry into the cabinet controlled by the NSS.
This action has been completed.
Action Taken to Provent Recurrence 1.
Following the completion of the above corrective actions, the Nuclear Station Operations Supervisor has made spot checks on the NSS key cabinet and has found the cabinet to be locked at all times.
2.
All Operations shift personnel will attend a review of Technical Specification 6.12.2, High Radiation Area, and Operating Manual Chapter 1.48.5.C, Padlock / Locking Devices Administrative Requirements, to be included in their next licensed retraining or non-licensed retraining module.
The Administrative Assistants, who are not required to attend retraining, will be counseled on the violation and action to prevent its recurrence.
The Administrative Assistants will also be given a
copy of the training package used in the retraining module.
This action will be completed by December 8, 1986.

Each Operations sheervisor authorized to issue keys under NSS control was given a
copy of the NRC Inspection 50-334/86-18 including Appendix A, Notice of Violation for their review.
This action has been completed.
Date on Which Full Compliance Will Be Achieved Beaver Valley Unit 1 is currently in full compliance with Technical Specification 6.12.2, High Radiation
- Area, and Operating Manual Chapter 1.48.5.C, Pailock/ Locking Devices Administrative Requirement.
The actions remaining are administrative in nature and will be implemented by December 8, 1986.
