DCb g 16 \lil MODIFICATION NO. 7 TO COOPERATIVE AGREEMENT NO. NRC-04-97-064 BETWEEN UNIVERSITY OF-WISCONSIN ~AT MADISON CENTER FOR HUMAN PERFORMANCE IN COMPLEX SYSTEMS AND THE U.S. NUCLEAR REGULATORY COMMISSION The purpose of this modification is to extend the period of performance through December 31, 1999 at no additional cost to the Govemment. Accordingly, the following change is hereby made:

Block No. 3, Cooperative Agreement Period, is deleted in its entirety and replaced with the following:
"FROM: 09/08/97 TO: 12/31/99" A summary of obligations for this Cooperative Agreement, from award date through the date of this action, is given below:
Total FY97 Obligation Amount: $325,000 Total FY98 Obligation Amount: $ Total FY99 Obligation Amount: $ 16,173 Total NRC Obligations: $341,173 This modification obligates FY99 funds in the amount of $ -0 .
O All other terms and conditions, including the Cooperative Agreement ceiling amount of
$341,173, remain the same. /
