9 NOTICE OF VIOLATION Illinois Power Company License No. CPPR-137 As a result of the inspection conducted on April 8 through May 12, 1986, and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the following violation was identified:

10 CFR 50, Appendix B, Criterion V, as implemented by the Illinois Power Company Operational Quality Assurance Manual (0QAM) requires that activities affecting quality shall be accomplished in accordance with written instructions, procedures, and drawings.

Procedure CPS No.1029.01, Revision 8, Preparation and Routing of Maintenance Work Request (MWRs), requires:

a. In Paragraph 8.2.24 that the Maintenance Group Supervisor evaluate the MWR for adequacy of corrective action taken and work completion.
b. In Paragraph 8.2.26 that any MWR that has been designated "Q" MWR shall be forwarded to Quality Assurance for review upon completion of the work, prior to final closeout of the MWR.
c. In Paragraph 8.2.16 that if during the performance of the work it is determined by the Maintenance Group Supervisor that a change in scope on a Quality Related MWR is required, the work package is returned to planning.
d. In Paragraph 8.2.10 that for Section XI repair or replacement criteria, the MWR is reviewed by the ISI Repair Coordinator.

Contrary to the above, activities affecting quality were not accomplished in accordance with CPS No. 1029.01 in that:

a. MWR B-27868 had work performed under the MWR, as documented in the remarks section, that did not address the problem described. In addition, the post maintenance functional testing required by the MWR job steps was not documented as having been performed. Thus the work and post maintenance testing associated with the MWR was not completed but the MWR was signed as complete by the Maintenance Group Supervisor,
b. "Q" MWRs B-35651 and C-05923 were closed and vaulted without a review by Quality Assurance af ter completion of work.
c. MWR C-03982 was closed and vaulted with unauthorized work performed without routing the MWR back to planning when a change in scope was required.

d. MWRs B-31300, B-31305, and C-05024 were not reviewed by the ISI Repair Coordinator for Section XI repair or replacement criteria.

This is a Severity Level IV violation (Supplement II).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation: (1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Consideration may be given to extending your response time for good cause shown.
