Dear Mr. Lofy:

The purpose of this modification is to: (1) revise the period of performance by revising the end date from September 26, 1997 to October 31, 1997; (2) increase

.the task order obligated and ceiling amounts by $27,078.11 from $433,857.99 to

$460,936.10; and (3) revise the Statement of Work (S0W) as indicated by highlights on the attached revised 50W.

In general, changes to the following headings in the 50W are:

Work Reauirements and Schedule Task 1 is revised to change the dates for the review of the implementation of the approved ICAVP audit plan for Millstone Unit 3; Task 2 is revised to change the dates for the review of the implementation'of'the approved ICAVP audit plan for Millstone Unit 2; Task 3 is revised to change the dates for the performance of the vertical slice system review at Millstone Unit 3; Add Task 4 - Review the Millstone Unit 3 system boundaries for the systems selected for the ICAVP contractor's Tier 1 system review.

Add Task 5 - Provide technical support to the NRC at the Millstone site by reviewing items for closeout on the NRC's Sito issues List (SIL).

Report Reouiremenh Under the heading " Technical Report," delete the paragraph in its entirety and replace it with the following:

"During Tasks 1, 2, 3, 4, and 5, the contractor's specialist shall provide daily reports to the NRC Team Leader or as directed by the NRC Technical Monitor. At the completion of Tasks 1, 2, and 3, the contractor shall provide a final summary report to the NRC Team Leader. At.the completion of Task 5, the contractor shall provide a final summary report to the NRC Technical Monitor.

The format and scope of these reports shall be as directed by the NRC Team Leader or Technical Monitor."

Task Order No.127 shall be in effect from July 16, 1997 through October 31, 1997, with-a cost ceiling of $460,936.10. The amount of $448,661.60 represents the estimated reimbursable costs, the amount of $160.72 represents the facilities capital cost of money, and the amount of $12,113.78 represents the fixed fee.

- PERFORMANCE PERIOD: 6/30/1997 to 10/31/1997 BACKGROUND On August 14,1996, a coni,,iiiatory order was issued requiring completion of an independent Corrective Action Verification Program (ICAVP) before rastart of any Millstone unit. The order directs the licensee to obtain the services of an organization, independent of the licensee and its design contractors, to conduct a multi-disciplinary review of Millstone Units 1,2, and 3., Tt,ie review is to provide independent venfication that the licensee's Configuration Management Plan has identified and resolved existing problems, documented and used licensing and design bases, and established programs, processes and procedures for effective configuration b

management in the future.

- The Director of NRR has established a team, headed by an SES-level manager, that is responsible for overseeing the performance of the ICAVP. The objective of the ICAVP Oversight Team is to assure that the review conducted by the ICAVP contractor is independent of the licensee and its design contractors, performed by qualified individuals, and is

- comprehensive, incorpoi.iing appropriate engineering discipline and operational reviews. The results of the ICAVP system reviews should provide the NRC with the insights necessary to 7

determine whether NNECO has been thorough in the identification and resolution of design-related problems for the Millstone units; 4

The NRC's ICAVP oversight staff will perform a number of tasks to ensure that the licensee's -

configuration management corrective action programs, and the ICAVP, have been effectively -

implemented. These tasks include the (1) review and approval of the contractor, and individual' y contractor specialists, selected by the licensee to perform the ICAVP; (2) review and approval

- of the contractor's audit plan for performing the ICAVP; (3) independently assessing the Llicensee's implementation of its configuration management corrective action programs; (4)

assessing the performance of the ICAVP contractor's implementation of the ICAVP; (5)

- monitoring the contractor interactions with the licensee as specified in the approved

communication protocol to ensuie continued independence from the licensee; and (6) evaluating the_ adequacy of the licensee's corroclive actions, and their implementation, including

corrective actions ' developed in response to the ICAVP contractor's findings and recommendations, and the findings from'the NRC's ICAVP oversight. Each of these tasks will-require engineering support to varying degrees. New Task Orders will be prepared to support performance of each of the remaining tasks above individually or in groups in the future.

OBJECTIVE The oojective of this Task Order is to obtain expert technical assistance (2 electrical, 2 y mechanical, 2 instrumentation and control, and 2 piping design engineers) in the performance of a review of the ICAVP contractor's implementation of the approved audit plans for the ICAVP at Milistone Unit 3 and at Millstone Unit 2, and for the performance of a vertical slice system inspection at Millstone Unit 3. The engineers should primarily have a design background, such as from an architect-engineer firm, but should also be familiar with installation and testing and site engineering organization. The engineers should be thoroughly familiar with NRC regulations and inspection methodology.

NRC FURNISHED MATERIAL Documents required to prepare for the inspection will be provided by the NRC Team Leaders.

OTHER APPLICABLE INFORMATION The work specified in tasks 1,2, and 4 of this SOW is not fee recoverable. The work specified in tasks 3 and 5 of this SOW is 100 percent fee recoverable. The contractor shall provide fee recovery information in the monthly progress reports in accordance with the basic contract. The.

Inspection report number for the work performed under tasks 3 and 5 will be provided by the NRC's Technical Monitor prior to the start of the task 3 or 5 activities.

The contractor's specialist assigned to this task order will have to be badged for unescorted access privilege at the plant site. The contractor shall provide all documentation required for badging (as identified by the NRC Team Leader) at the plant site. Questions concerning badging and the plant site access shall be addressed to the NRC Technical Monitor.
