An Operating examination was administered to one SR0 candidate.
A review of the candidate's written requalification exam administered by the facility was performed with no adverse findings.

Waiver of Written Examination:
The written examination was waived. The candidate had a previous Itcense at the facility. His license had just recently expired after the individual had taken a position with another firm for a short time.

The examiner met with the licensee representative denoted in Paragraph 3 above to discuss the examination process.
