This refers to Certificate of Compliance No. 5916 for the Model No. 2501 packaging which has an expiration date of April 30, 1987.
To ensure continued use of this package, an application for renewal (6 copies) of the existing certificate should be submitted at least 30 days prior to the expiration date of the certificate.
Unless an exemption is provided by 10 CFR Part 170.11, an application fee of
$150.00, as required by 10 CFR Part 170.31, shculd be included with the request for renewal. This notice of expiration should not be construed that such notice will be provided in the future. Also, please let us know if you do not intend to request renewal of the certificate.
