Dear Mr. Schopfer:
In reviewing the discrepancy reports (DR?) developed during your implementation of the independent Corrective Action Verification Program (ICAVP) at Millstone Unit 3, the Special Projects Office (SPO) staff of the Office of Nuclear Reactor Regulation (NRR) Identified a concern with how certain DRs were determined to be invalid. The concern relates to DRs that you had determined were invalid based on the relatively low significance of the issue described in the DR. Several examples of DRs that the SPO staff found to be dispositioned in this manner were discussed with you in a recent telephone conversation. The examples included DR MP3-0044 and DR MP3-0054.

In developing the four levels of DR r,gnificance that were provided to you in our July,15,1997, i
letter, the SPO staff had establisheJ the lowest level (level four) to capture relatively minor errors such as those described ' i the subject DRs. As such, the SPO staff has determined that it was inappropriate to characterize the minor errors discussed in the subject DRs as invalid based on a low significance. You will be informed as the SPO staff identifies additional examples. You are requested to review the DRs that you have determined to be invalid to l
identify others that you may have inappropriately characterized as invalid and as necessary make the changes to ensure the DRs are properly dispositioned, l
