The reporting period began and ended with the Unit in Mode 1 at 80% rated thermal power.

No significant (greater than 10 MW).

SUMMARY

9-4-86 At 0536, Train

'A' safety injection was declared inoperable to repair a body-bonnet leak in IMO-270 (T/S

9-5-86 At 0138, Train

'A' safety injection was declared operable following completion of repairs to-IMO-270.

9-12-86 At 1845, All four 250 VDC battery chargers were declared inoperable due to indeterminate seismic qualifications on the newly installed equalize charge.

T/S 3.0.3 was entered since the action of T/S 3.8.2.3 was exceeded.

At 1915, Unit shutdown commenced.

At 2158, The 2-AB-2 battery charger was declared operable after being placed in service with the timer electrically removed.

Exited T/S 3.0.3 with 3.8.2.3 action in effect.

2-AB-1 remains out of service.

At 2202, The 2-CD-1 battery charger was placed in service with the timer disconnected.

Exited.T/S 3.8.2.3 action with the 2-CD-2 charger out of service.

At 2205, Reactor power reduction was stopped after reduction of 3 4 MW.

At 2230, Reactor power was at 80%.

At 2310, The 2-AB-1 and 2-CD-2 battery chargers were declared operabic.

2 of 2 9-15-86 At 1400,'The NRC was notified that both trains of safety injection were inoperable.on 9-4-86 due to closure.of IMO-270 and IMO-275, with one pump out on clearance.

The underlying concern is that 2 of 4 loops required by the FSAR were blocked.

9-17-86 At 1003, The TDAFP was declared inoperable because FMO-211 and FMO-241 were closed after discovering leakage into the feedlines.

(T/S 3. 7.1. 2)

At 1335, The TDAFP was declared operable and FMO-211 and FMO-241 were opened, following determination that the check valves leaking-into the feedlines had been reseated.

4 9-19-86 At 0617, The east Component Cooling Water Pump was declared inoperable due to an oil leak.

(T/S 3.7.3.1)

At 1622,-The East Component Cooling Water Pump was declared operable.

l 9-23-86 At 1755,.The NRC was notified of a possible violation l

of shutdown margin, (T/S 3.1.1.1 and 3.'1.1.2) due to Boron --10 depletion.

The incident remains under investigation.

Plant conditions were determined satisfactory for continued operation.

Svensson TELEPHONE (616) 465-5901 PAGE 1 of 1 MAJOR SAFETY-RELATED MAINTENANCE SEPTEMBER, 1986 2-M-1 2-MRV-221, Steam Dump Valve for MRV-220, Number 2 Steam Generator Stop Valve, was leaking by.

Disassembled and cleaned valve.

Replaced gaskets, stem, plug,-seat ring, and pin.

Reassembled valve.

Valve tested satisfactorily.

2-I&C-1 WFI-736, essential service water flow to 2 west component cooling water heat exchanger indicator pegged high with only 2000 GPM flow on 2 west essential service water header.

Found transmitter failed high.

Replaced defective amplifier board and range potentiometer.

Calibrated transmitter and local indicator.
