SUBJECT: NRC INSPECTION REPORT 50 445/97 17,50 443/97 17 AND NOTICE OF
VIOLATION
Dear Mr. Terry:
On August 30,1997, the NRC completed an inspection at your Comanche Peak Steam
Electric Station, Units 1 and 2 reactor facility. The enclosed report presents the scope and
results of that inspection.
During the 6 week period covered by this inspection, your conduct of activities in all areas
was generally characterized by safe and conservative decisions and practices. However,
some problems were noted in each area inspected.
Three violations were identified during the inspection. The first violation involved three
exampled of failure to follow procedural requirements. Although each example had only
minor safety significance, we are concerned about a potential adverse trend in human
performance.- The second violation involved the failure to perform required Technical
Specification surveillance testing af ter performing maintenance on a containment isolation
valve actuator. :The third violation was cited for inadequate lighting in the protected area.
These violations are cited in the enclosed Notice of Violation and the circumstances
surrounding these violations are described ir, detailin the enclosed report. Please note that
you are required to respond to this letter and should follow the instructions specified in the-
enclosed Notice when preparing your response. The NRC will use your response, in part,
to determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter,
its enclosure (s), and your response will be placed in the NRC Public Document Room
(PDR). To the extent possible, your response should not include any personal privacy,
proprietary, or safeguards information so that it can be placed in the PDR without
redaction.
