Attachment I to CNRO 97/00088 Page1 Cf 2 Notice of Violation 9712 01 10 CFR Part $0, Appendix B, Criterion V, states, in part, that activities affecting quality shall be prescribed by documented procedures and drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these procedures and drawings.

Procedure 01 S-07-43," Safe llandling of Loose items inside the Plant," Revision

1. Paragraph 5.11, states that equipment; piping; conduit; cable trays; instrumentation; heating, ventilation, and air conditioning; and their associated supports are not considered structural elements and cannot be used for securing loose items without engineering approval.

Contrary to the above, on June 30,1997, the inspectors identified an extension ladder supported on a safety related cable tray and no engineering evaluation had been completed.

This is a Severity Level IV violation (Supplement 1)(50-416/9712 01).

I.

Admission or Denial of the Alleged Violation Entergy Operations, Inc. admits to this violation.

II.

The Reason for the Violation,if Admitted The cause of the violation was plant personnel's insufficient awareness of the impact on safety / reliability when utilizing a ladder. Contrary to procedure, plant personnel assumed an evaluation was only required if the ladder was left in place unattended vice the evaluation required before placing the ladder against a safety related cable run.

Imnsediate Corrective Steps Which llave Hecy,Taken and Results Achieved The ladder was promptly removed upon notification of non-conforniance.

A deficiency document was generated to address the issue.

IV.

Corrective Steps Taken to Pretiude Further Violations Discussed Standard GGNS-CS-17 with maintenance personnel and the importance of procedure adherence in regard to the standard, Tasks associated with this event have had a note added to the cover sheet stating that e

a loose items evaluation is required when performing task.
