P. Charlie PURPOSE:
To conduct a review of construction activities at the Shiprock UMTRA Project for assessment of whether the quality control program is functioning in a manner that assures compliance with the RAP and the EPA standards.
ENTRANCE INTERVIEW Following an introduction of all participants, the NRC staff indicated that the review would consist of a tour of the site, observation of 3

-2 IlAY 3 01986 of remedial action construction and the activities presently in progress and indicated that he.and his QA supervisor-(John Roesch) would be available to accompany the NRC staff on the site tour.
SITE TOUR The RAC personnel conducted the NRC representatives and the other participants on a tour of the site. Activities observed include the following major work areas:
Placement and compaction of the radon barrier (in final stage of completion, approximately 60 percent complete).
Other aspects of the project that were observed include the following:

Four _(4) rock grab samples were collected for the purpose of gradation tests.

Stabilization of the arroyo area at the east end of the pile is
-presently on hold.

The escarpment at the east end of pile is presently being reconstructed and stabilized.
5.
Decontamination facility is shut down.
6.
Temporary fence is being relocated (to south and east perimeter).
7.
Settlement plates are located at the east end (3) and west end (3) of the pile.

Rock erosion protection placement will begin about the second week of July, 1986.
TESTING OBSERVATION During the site review, the staff observed quality control personnel during their performance of field moisture / density testing of the radon barrier. A nuclear density test (troxler) was run on a central area of the pile. The quality control personnel appear knowledgeable in all aspects of their testing and inspection duties and followed appropriate OFC -:

_3-MAY 3 01986 procedures in performing the testing observed. The results of the density tests met the specifications, but the moisture reading was slightly above the specified upper moisture content limit.
RECORD REVIEW The staff selectively reviewed testing and daily inspection records and found them to be complete and to verify performance of construction in accordance with applicable specifications. Reviews were conducted on the following records:
1.
Gradation tests.
2.
- Sand cone and nuclear density tests.

Proctor dynamic test.
4.
Instrument calibrations / frequency (nuclear density gauge).
5.
Laboratory equipment calibration / frequency (scales, meters,
- ounters, and cone).
6.
Staff qualifications.

EXIT INTERVIEW:
Prior to leaving the site, the NRC staff met with 00E, TAC and RAC personnel to discuss issues identified during the site review.
ISSUES:
m Gradation testing will be conducted on rock type (s) B and B1 (two samples of each type). This is to acquire a more representative gradation measurement for the described rock type (s).
