June 21, 1999 ALL AGREEMENT STATES MINNESOTA, OHIO, OKLAHOMA, PENNSYLVANIA, WISCONSIN TRAINING COURSE INFORMATION: ACCEPTANCE TO THE SAFETY ASPECTS OF INDUSTRIAL RADIOGRAPHY COURSE (H-305) (SP-99-041) is the list of students from the States selected to attend the August 9 - 13,1999 Safety Aspects of Industrial Radiography Course (H308). Please provide the list of students and the instructions (Enclosure 2) to each individual from your program who is on the list. States with students attending this course have agreed to pay travel expenses, and, where designated, the tuition. To cover the tuition expense, each State should submit a purchase order or equivalent document, payable to the U.S. Nuclear Regulatory Commission, to the following address:

The NRC will, in turn, issue you a bill (invoice). If for any reason your attendee is unable to attend, please submit to us a written purchase order, or equivalent, to cancel, prior to the starting date of the course.

Course attendees should be encouraged to make travel and lodging arrangements as soon as possible. If for any reason a candidate is unable to attend, please contact us as soon as possible, so that alternative arrangements can be made.

ACCEPTANCE: This is to advise you that those individuals in Enclosure 1 have been accepted for participation in the training course (H-305) Safety Aspects of Industrial Radiography." This course is scheduled to be presented August 9 - 13,1999 at Hellier Associates,277 West Main Street, Niantic, Connecticut 06357; Telephone (860) 739-8950.

' LODGING AND TRAVEL: Attached is a list of hotels in the vicinity of Hellier Associates. You should plan to arrive on Sunday, August 8,1999 and depart on Friday, August 13,1999.

Participants must make their own lodging arrangements. Individuals should request a State or govemment employee rate at the hotels.

New York City Area - Follow l-95 North to Exit 72 (Rocky Neck State Park Connector). Follow connector to end and turn right on W. Main Street (Rt.156). Hellier Associates is located approximately 4/10 of a mile from intersection on right side of road (first driveway past restaurant but before residential area). Hellier Associates is located through the center door of the brown brick building.

New London /Groton Airport - When you come out of the airport, take a left to Tower Avenue which changes to High Rock Road. At end of High Rock Road, turn left to Poquonnock Road.

At first traffic light, tum left to Rainville Avenue. At the next light, tum right on Clarence B. Sharp Highway (Rt. 349) and continue straight and you will see signs for I-95. Exit for l-95 South will be on the left side of the road. Follow l-95 South to Exit 72 (Rocky Neck State Park Corinector).
