During the March 1986, investigation of circumstances surrounding an incorrectly hung "out-of service" on the Unit 2 High Pressure Core Spray system it was determined that a Nuclear Station Operator (NS0) had entered the initial of an Equipment Attendant on a checklist resulting in a second verification of a breaker position not being performed. Based on this determination, the NSO was removed from licensed duties. On May 12, 1986, an enforcement conference was held at the NRC Region III office to discuss this matter. On June 19, 1986, NRC. Region III confirned commitments made by you at the May 12, 1986, conference, regarding an audit program whereby the NSO would be monitored for satisfactory performance following his return to licensed duties. Subsequent to the NS0's restoration to licensed duties the individual made another error in verifying an "out-of-service" and was again removed from licensed duties.

On January 16, 1987, Comonwealth Edison Company submitted a letter to NRC Region III identifying a desire on the part of the company to pursue once again returning this individual to licensed duties. This letter delineated the basis for this renewed effort and presented a four phase plan to return the NSO to licensed duties.

My staff has reviewed this prcposal and the associated four phase plan. We fir.d both to be acceptable. On this basis we are hereby modifying our June 19, 1986 Con #irmatory Letter to incorporate by reference the four phase plan detailed in Comonwealth Edison Company's January 16, 1987 letter.
