OF TELEPHONE CONVERSATIONS WITH i

ALLEGER CONCERNING SPENT FUEL RACKS On January 13, 1987, an individual contacted the duty officer at the Inspection l

and Enforcement Operations Center. The individual identified himself as an ins sector working for a utility et a vendor shop that was fabricating spent fuel racts. He stated that he was reemved from his job after raising safety concerns regarding the fabrication of the racks. The individual identified the vendor as U.S Tool and Die located outside Pittsburgh in Allison Park, Pennsylvania, but did not offer any other information.

The individual later spoke to Gene Holler and Barry Zalcman, who advised him of his rights under 10 CFR Part 50.7 and possible Department of Labor (DOL) i involvement. The individual was concerned about confidentiality and spoke to them about it.

Ellis Merschoff was contacted by the duty officer and informed John Craig that l

an individual may have allegations concerning fuel racks. John Craig and I called the individual to obtain specific information regarding the possible allegation. The individual again identified himself as a utility inspector at l

a vendor shop that was fabricating safety related equi > ment for a utility. He had recently been fired by the utility and felt that t11s was a result of his raising safety concerns. He stated that during his employment he observed vendor employees not following procedures and he had identified defective welds.

The individual declined to reveal information regarding the name of the utility or vendor and was concerned about confidentiality.

On January 28, 1987, John Craig and I called the individual again asking for more specific information. He stated that he had decided not to contact DOL, but did identify the utility as Wisconsin Public Service Corporation and the vendor as U.S. Tool and Die. He again stated that the company was not following procedures and was production orientated, not quality orientated. He noted that in one instance af ter observing defective welds, rewe' ding was perfonned. After the rewelding, he questioned vendor employees about any problems which might have been identified concerning the weld quality and was informed that no problems were found. The individual then asked a welding inspector from Gilbert Comonwealth to review the weld inspection documentation and examine the welds.

The inspector identified defects in the welds. When the utility and vendor were questioned regarding the defective welding and inspection, both denied performing any type of inspection and no documentation was found.
