AND NOTICE OF VIOLATION

On July 27,1997, the NRC completed an inspection at your Three Mile Island 1 & 2

.

reactor facility. The enclosed report preshnts the results of that inspection.

I

The inspector noted that your staff's overall performance was good in responding to the

g

electricalissues identified after the loss of offsite power (LOOP) event. Your staff had

replaced the two damaged substation main generator output breakers with new suitable

I

breakers. The inspector noted that your staff had planned to determine the cause of

unbalanced current condition observed in these breakers by a substation technician prior to

the LOOP event on June 21,1997. Even though your staff had planned to troubleshoot

the condition, this issue was not proper ly documented per your corrective action process

l

and plant management was not made aware of this anomaly prior to the LOOP event.

Therefore, we are considaring this an unresolved open item until our final review is done.

Plant personnel and equipment response to the June 21 reactor trip and los of offsite

y

power were excellent, especially in light of the previous 617 day continuous run.

!

Management applied a conservative approach to safe plant operation when they decided to

o

remain in a heightened emergency response mode until the reactor coolant pumps and the

[

main condenser heat sink were restored to service. Excellent coordination and replacement

- of the faulty main generator output breakers was noted.-

However, the required notifications to the Commonwealth and risk counties for the June

i

21 Unusual Event were not completed within 15 minutes as required by GPU emergency

/

plan implementing procedures and NRC regulations. The violation for the late notification

I

is cited in the encic3ed Notice of Violation, and the circumstances surrounding the violation

i

are described in detailin the enclosed report. This is a repeat problem that was also noted

)

in two emerger..:y plan exercises in the past 15 months. Additional reinforcement of the

.

I

importance of the emergency planning program is warranted to ensure that the ability to

implement the program is maintained. Please note that you are required to respond to this

j

'

'etter and should follow the instructions specified in the enclosed Notice when preparing

your responso. The NRC will use your response, in part, to determine whether further

enforcement action is necessary to ensure compliance with regulatory requirements.

The TMI radiological controls occupational exposure program and radiological

environmental monitoring program implementation continue to be effective. Both programs

included strong management oversight and effective quality assurance audits that assessed

program strengths and weaknesses. The radiation protection department performed

numerous self assessments and program evaluations of significant scope,

in accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter

and its enclosures will be placed in the NRC Public Dc:ument Room (PDR).
