NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-254 As a result of the inspection conducted on November 30, 1986 through January 31, 1987, and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1085), the following violation was identified:
Technical Specifications Section 6.2 requires that detailed written procedures, including applicable checkoff lists, shall be prepared, approved ar.d adhered to for normal startup, operation, and shutdown of the reactor, and other systems and components involving nuclear safety of the facility.
Licensee procedure QGP 1-1, " Normal Unit Startup," directs the performance of procedure QOP 5400-1, "Offgas System Startup." QOP 5400-1 requires the lineup of the primary steam jet air ejectors at 400 psig reactor pressure on a startup.
Contrary to the above, on December 9, 1986, at 1733 hours0.0201 days <br />0.481 hours <br />0.00287 weeks <br />6.594065e-4 months <br /> Unit 1 scrammed on low condensor vacuum due to failure to adhere to procedure QGP 1-1, " Normal Unit Startup," Rev. 38. Specifically the steam supply to the primary steam jet air ejectors was not valved in as specified in the procedure.
This is a Severity Level IV violation (Supplement 1).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation:
(1) correctiva action taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
