Enclosed is the NRC license amendment which you requested.

Please review the enclosed document carefully and be sure that you understand

all conditions. If there are any errors or questions, please notify us so

that we can provide appropriate corrections and answers.

You should note that 10 CFR 20.207 requires that licensed materials be

secured from unauthorized removal from the place of storage. Rooms 19 and 24

'should be secured whenever licensed materials are stored there and the

authorized user is not present.

Also, note that the application dated February 15, 1982 specifies that the

survey procedures in Appendix I of Regulatory Guide 10.8, Revision 1 will be

followed. Appendix I states that all elution, preparation, and injection

areas will be. surveyed daily.

Finally, you should submit a survey of the area previously used for nuclear

medicine procedures. Requests for releases of facilities where licensed

materials have been used should be submitted to NRC Region V for

consideration. These requests are reviewed on a case-by-case basis. Areas

-

should not be released for unrestricted use until Region V has concluded that

the decontamination efforts meet the criteria in the July 1982 guidelines.

In some cases, confirmatory surveys by Region V staff will be required.

Any future correspondence relating to your license should specifically

reference your license and docket numbers to expedite your inquiry.
