SYSTEMATIC ASSESSMENT OF LICENSEE PERFORMANCE (SALP)
REPORT 50-333/97-99
Dear Mr. Knubel:
This letter forwards the SALP report for the James A. FitzPatrick Nuclear Power Plant for the period November 19,1995, through June 28,1997 (Enciosure 1). This assessment was conducted under the SALP process that was implemented by the Nuclear Regulatory Commission (NRC) in July 1993. In this process, the NRC rates the performance of licensees in four functional areas: Operations, Maintenance, Engineering, and Plant Support. The Plant Support area comprises radiological and effluent controls, chemistry, security, emergency preparedness, fire protection, and housekeeping.
Performance at FitzPatrick was generally good with improvement in the plant material condition. Self-assessments were generally effective. Some improvements were noted in problem reporting. However, human performance problems, involving each of the principal plant organizations, continued from the last SALP. Several personnel errors stemming from a lack of attention to detail and the failure to exhibit a questioning attitude directly impacted on plant operations. Work control weaknesses resulted in challenges to plant operations. Continued close management attention is needed to improve performance in these areas.
Performance in the operations area was rated Category 2. Operations management provided generally good oversight of plant activities and demonstrated a conservative approach to operation of the plant. Significant attention was given to reducing operator workarounds and burdens. Operator performance and response to plant transients was generally good. However, personnel errors and problems with the control of plant configuration continued to be a concern.
Performance in the maintenance area was rated Category 2. Good performance was

achieved as evidenced by the maintenance work request backlog reduction, and generally a _ssa improved material condition and equipment reliability. However, personnel errors during E!B.

good self assessment efforts to identify engineering performance improvement opportunities were noted. Engineering efforts were effective in reducing operator workarounds. However, there were instances in which formal configuration controls were bypassed and safety evaluations were not performed as required. While efforts have been made to review and improve retrievability of design basis information, some lapses in tracking related corrective actions occurred.
Performance in the area of plant support was rated Category 2. Continued effectiveness of emergency preparedness, fire protection and security programs was noted. The occupational radiation protection program was generally effective. However, several noteworthy problems occurred involving the failure of radiation workers to adhere to radiological control barriers and procedures, a recurring problem from the previous SALP period. These problerr; persisted despite initiatives undertaken early in the period to improve radiation worker and radiation protection technician training and performance.
We have tentatively scheduled a management meeting with you at 10 a.m. on September 4,1997, at the FitzPatrick site. The meeting is open to the public. At the meeting, you should be prepared to discuss our assessment and the initiatives you have ongoing or plan to take to address the weaknesses detailed in our evaluation. provides the schedule of NRC inspections of your facility planned for the next year. We willinform you of any changes. The routine resident inspection effort is not included in this schedule. We oppreciate your cooperation.
