NRC INSPECTION REPORT 50-213/97-06

Dear Mr. Feigenbaum:

This refers to the safety inspection conducted by Messrs. R. L. Nimitz and W. J. Raymond

of this office during the period June 16 - July 18,1997, at the Connecticut Yankee Atomic

Power Station, Haddam, Connecticut. The encloscd report presents the findings of this

inspection. Mr. Nimitt discussed the findings with Mr. Hasseltine of your staff and others

on June 20,1997, and also discussed the findings with Messrs. Mellor, Bouchard, and

Sexton during a June 27,1997, telephone conversation, in addition, the overall

conclusions of the inspection were discussed during a July 18,199',, meeting held at the

NRC Region i Office, King of Prussia, Pennsylvania. This meeting was open to the public.

Areas reviewed during the inspection are discussed in the enclosed NRC Inspection report

and included radiological controls for diving activities, planning and preparation for

shipment of non-irradiated fuel, and applied radiological controls.

The inspection identified several examples of failure to meet license requirements, as

described in the enclosed Notice of Violation. The circumstances surrounding the

violations are described in detailin the enclosed report. The violations concern the failure

by plant personnel, specifically radiation protection personnel, to follow procedures. The

findings represent additional examples of failure of station personnel to implement applied

radiological controls requirements and are similar to those examples identified during the

November 2,1996, fuel transfer event, in addition, the recent examples indicate

corrective actions taken to improve performance have not been fully effective and

personnel do not fully understand procedure requirements. Further, supervisors do not

readily detect and correct problems associated with personnel adherence to procedures.

Our resident inspection personnel noted similar procedure adherence concerns in other

program areas at the station.

At the management meeting held on July 18,1997, you presented plans anc actions to

improve performance in the area of personnel adherence to procedures. Those actions are

described in the attachment to the enclosed inspection report. We will cont.nue to monitor

overall station performance in this area and the adequacy and effectivenest of your

corrective actions.

You are required to respond to this letter and should follow the instructions specified in the

enclosed Notice when preparing your response. The NRC will use your response, in part,

to determine whether further enforcement action is necessary to ensure compliance with

regulatory requirements. In addition, and as a result of the NRC observations in the area of

receipt and shipment of new fuel, please include in your response a description of your

actions, taken or planned, to assure that personnel who are expected to handle radioactive

materials are adequately trained and qualified in accordance with 49 CFR 172, Subpart H.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter,

its enclosures, and your response will be placed in the NRC Public Document Room (PDR).

To the extent possible, your response should not include any personal privacy, proprietary,

or safeguards information so that it can be placed in the PDR without redaction.
