Sellman President Maine Yankee Atomic Power Company 329 Bath Road Brunswick, Maine 04011 SUBJECT: NRC INTEGRATED INSPECTION REPORT 50-309/97 06 AND NOTICE OF VIOLATION Dear Mr. Sellman: On July 27,1997, the NRC completed an inspection at your Maine Yankee reactor facility. The results of that inspection were discussed with Messrs. G. Leitch, R. Fraser, R. Blackmore and others of your staff at an exit meeting held on July 30,1997. The enclosed report presents the results of that inspection. Weaknesses were noted in operator performance as evidenced by tne valve line up errors that resulted in an unplanned transfer of water from the upender pit to the refueling water storage tank. The decision to institute an operations stand down in response to these errors was good management intervention and reflected good safety focus. However, after the stand down was completed, the NRC identified a poorly implemented temporary procedure change. -This indicates that the corrective actions were not fully effective in establishing the expected performance standards. Your continued management attention -to this area is warranted. In the area of plant support, we found that adequate health physics (HP) support and radiological control preparations were provided for planned work. One violation of NRC requirement was identified during this inspection. The violation involved fai!ure to adhere to plant procedures on two separate occasions that resulted, on , ' one occasion, in an unplanned transfer of 10,000 gallons of water to the Refueling Water Storage Tank, and on the other occasion, in a trip of the Spent Fuel Pool Purification Pump. This violation is cited in the enclosed Notice of Violation, and the circumstance surrounding the violation is described in detail in the enclosed report. Please note that you are required , to respond to this letter and should follow the instructions specified in the enclosed Notice when preparing your response. The NRC will use your response, in part, to determine whether further enforcement action is necessary to ensure compnance with regulatory requirements.
