Your letter should identify the individuals who will take the examination, The letter should certify I
that they are :nrolled in a facility licensee-sponsored program leading to an NRC operator or senior operator license and that they will have completed their furdamentals training by the date of the examination. _ The letter should also identify the personnel who will have access to the examinations before they are administered (e.g., proctors) and the address to which the -
examinations are to be sent. The letter shou'd be received in both the NRC Regional and Headquarters' offices 30 days before the examination date to allow the NRC to aseign docket numbers. A sample registration letter is enclosed.
