- During this reporting period, Constuners Energy conducted a total of 412 random tests with the following results:

PRE-ACCESS TESTING -

A total of 138 pre-access tests were conducted, resulting in two (2) positive determinations for L

THC. Both were short-term contractors at Palisades. Access was denied to both individuals.

C.

FOLLOW-UP TESTING A series of follow-up tests resulted in a second positive cocaine determination on an employee who worked in the support group. The employee's access was terminated. No appeal was made.

- D.

FOR-CAUSE TESTING One for-cause test was initiated on a licensed operator at the Big Rock Point Plant. Upon receiving credible. information from an 'outside source of the employee's alleged use and distribution of illegal substances, agreement was made to conduct the test.

A positive determination for THC resulted in immediate revocation of access. This individual requested a

. retest of his original specimen which was performed at another approved laboratory. The positive determination for THC was upheld. An internal hearing is in process.

INITIATIVES TAKEN/IREMONS 1RARNRD tan incident involving for-cause testing on a licensed operator which resulted in a positive test determination prompted a decision to have an independent-review of the Fitness For Duty -

- Program. Overall, the review identifial the program as meeting the intent of 10CFR26; however, corrective actions were necessary in the following areas:

.On-site FFD -Administrators did not always contact the randomly selected individual and i

supervisor, as directed by procedures, to determine if; hose selected are on site and available for i

FFD testing. As a result, the aforementioned operator was excused from a random test when in fact he was working at that particular day and time.- This error occurred when a work schedule

- was checked as opposed to contacting the operator's supervisor as to his availability.

. Supervisors were not requit d to sign " Excuse From Testing" forms, but were signed off by the site administrators instead, hocedures allow'. telephonic excuses to be documented by site administration, however, in some cases, supervisors were not contacted at all. Corrective action will require supervisors to sign all excuse forms and not allow telephonic excuses. In addition; individuals randomly selected and unavailable during backshifts, weekends / holidays will not 2-

immediately be yxcused. An effort will be made to reschedule collections in order to conduct testing on the majority of those identified.

The licensed operator was not escorted to the off-site clinic when sent for the for-cause collection process. --Although procedures do not require an escort, it was determined that a better practice in this case would have been to keep the operator under constant observation and to accompany him to the collection site.

RFPORTABIR EVENTS UNDER 10 CFR 26. SECTION 26.73 One reportable event, involving a Big Rock Point licensed operator testing positive for THC

- through the for-cause process occurred during this reporting period. The operator was removed from licensed duties at the time of his test, then revoked and terminated from employment upon notification by the MRO of the positive test result. Notification was made to the NRC Operations Center and courtesy notification made to NRC's Region III.

A thorough review was made of the operator's work dating back to when he received his license (August 1996).- Log books, periodic and scheduled tests were also reviewed tc evaluate plant events with respect to the operator's performance. All reviews resulted in a determination that i

the operator's performance was normal and signs of aberrant behavior were not evident, c
