SUBJECT: ~ NRC INTEGRATED INSPECTION REPORT 50 309/97-05 AND NOTICE OF

VIOLATION

Dear Mr. Sellman:

On June 8,1997, the NRC completed an inspection at your Maine Yankee reactor f acility.

The results of thU inspection were discussed with Messrs. G. Leitch, R. Blackmore and

others of your su.rf at an exit meeting held on June 12,1997. The enclosed report

< presents the results of that inspection.

During the 6 week period covered by this inspection period, activities at the Maine Yankea

facilities were conducted safely. However, a sequence of events during core offload

revealed some weaknesses in several areas of Maine Yankee performance, particularly

work control, testing, and problem identification and resolution. Continuing problems with

th9 performance of refueling equipment, particularly the spent fuel pool crane, indicates

that corrective actions for previous problems were inadequate or narrowly focused.

As part of our activities we conducted inspection of your laservice Testing (IST) Program.

Our inspection identified a number of deficiencies in your program. We noted that you

have on going efforts to identify and resolve probims with the IST program as a part of

your restart plan. Resolution of the findings of yo' r program review as well as those

identified in this inspection will be evaluated as a part of the NRC's restart plan.

Five violations of NRC requirements were identified during this inspection. The violations

involved
(1) the failure to adequately test refueling equipment prior to use;(2) the f ailure

to implement adequate corrective actions for fuel movement mishaps; (3) implementation

of alternative inservice test methods prior to obtaining NRC authorization: (4) inadequate

use of newly established vibration data for safety refeted pumps; and, (5) lack of adequate

test acceptance criteria for testing safety related pumps.

surrounding the violations are described in detailin the enclosed report. Please note that l

. you are required to respond to this letter and should follow the instructions specified in the -

enclosed Notice when preparing your response. The NRC will use your response, in part,

to determine whether further enforcement action is necessary to ensure compliance with

regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter

and its enclosure will be placed in the NRC Public Document Room.
