This Potentially Reportable Significant Deficiency Report is being submitted pursuant to the requirenents of 10CFR50.55(e).

It is anticipated th at an additional report will be issued by April 15, 1987.

Summary While performing a pre-service exanination (ultrasonic) of the regenera-tive heat exchanger girth welds, two welds were found to contain ultrason-ic reflectors requiring re-ev aluation.

The indications 'on these welds were identified, repaired and accepted in accordance with the existing quality progran.

During the course of the initial-investigation, a review of the radiographs furnished by the vendor for these welds. was perfonned,-

in an attenpt to correlate the ultrasonic. indications with the. indications on the raciographs.

During the course of this review,'these welds were found to have linear indications which would appear to be unacceptable to ASME Section III.

As a result, the renaining radiographs on the regenera-tive heat exchanger were reviewed.

Two additional _ welds were found to have areas which would appear to be unacceptable -to ASME - Section III.

During this initial review, a total of four (4) welds were found to have-linear indications.

Immediate Action Taken A Nonconformance and Disposition Report was initiated to identify Duquesne Light's concern regarding the indications found on the vendor supplied radiographs, and to detennine the extent of the inconsistencies in dates in the supporting documentation.

Mr. L. M. Rabenau, Compliance Engineer - Beaver Valley Unit 2, notified Mr. J. Shedlosky, NRC Region I of this concern on Decenber 30, 1986.

Description of Deficiency Two of the welds referenced aoove which appear to have linear indications on tne radiographs, were re-radiographed by DLC.

One of these welds was detennined to have lack-of-fusion.

The other weld was acceptable.

The other two welds have not been re-radiographed due to tube bundle interfer-ence.

The inconsistencies noted in the supporting documentation, supplied by Joseph Oat and Son, Inc., include itens such as reader sheets being reviewed and approved prior to radiograph dates, and the N-1 Data Report being signed prior to all welds being radiographed..

Analysis of Safety Implication j

The vessel was f abricated in accordance with the ASME Boller and Pressure j

Vessel Code,Section III,1974 Edition, Subsection NC.

As such, a lack-j of-fusion constitutes an unacceptable weld.

For the inconsistencies in the supporting documentation, the impact on 4

safety will depend upon the outcone of review of these inconsistencies.

Corrective Action to Resolve the Deficiency The subject Nonconformance and Disposition Report is currently in the evaluation stage for engineering disposition.

The outcone of the documen-tation review will be a detennining factor for the disposition.
