In reference (a), Quad Cities provided plans and actions to improve the material conditicn of the plant, which included replacement of the Source Range Monitoring (SRM) and Intermediate Range Monitoring (IRM) cables. In reference (b), the status of) those action plans were provided. At that time, I committed to replace the SRM/lRM / g1 cables on iJnit 2 during the Q2R!4 refueling outage. In reference (c), I reported that these cables were not replaced during the Q2R14 refueling outage and stated that an evaluation to determine our future plans concerning replacement of the SRM and IRM cables on Unit 2 would be completed.

Since 1993, several actions have been taken on both Unit I and Unit 2 to address the spiking problem, items such as the replacement of the under vessel connectors, improved detector and connector maintenance, improved testing techniques and replacement of preamplifiers. These measures have proven effective in reducing the spiking problems associated with the SRM/lRMs. During QlR14 the SRM/IRM cables were replaced. The replacement cables have not resulted in a quantifiable improvement in the reduction of spiking.
Therefore, we do not intend to install the SRM/lRM cables on Unit 2 as committed to in Reference A. It is our intent to continue monitoring these problems and to take appropriate steps to ensure spiking is minimized.
