CONTRACTOR PERFORMANCE REPORT AND CERTIFICATION OF PROJECT
. COMPLETION CONTRACT NO. NRC-33-92-342
'l In order to closecut this contract which expired on May 06, 1994, please provide the information requested on the attachment to this memorandum, and certify, within (15) calendar days, or as soon as practicable whether or not the final report / deliverable (s) submitted by the contractor are acceptable.
At NRC, the attached documents replace the " Notification of Receipt and Acceptability of Final Report / Deliverables / Government Property," which was completed by-the Project Officer for expired contracts prior to July 1995.
In accordance with Section 1091 of the Federal Acquisition Streamlining Act of f
1994, as implemented by Federal Acquisition Regulation 42.15, a " Contractor Performance Report" must be completed by the Project Officer and made available to the contractor for 30 days, for agreement, comments, or rebuttal.
The contracting officer then reviews the completed form and any comments provided by the contractor, and rates the contractor's performance.
This information must be made available to other Federal agencies seeking past-

performance information for procurement competitions.
- A separate " Certification of Project Completion" contains additional questions related to the administration of the closeout of this subject contract.
