This acknowledges receipt of NRC Form 241 dated July 17,1999, and the $1,100.00 application fee, which you submitted to report proposed activities in NRC jurisdiction under the authority of the generallicense pursuant to 10 CFR 150.20. Ycur Agreement State License No. IL-02068-01 is recognized as valid for the proposed use of licensed material as described in this initial application not to exceed 180 days in calendar year 1999.

We also acknowledge that licensed material is stored at your Portage, Indiana, site and is covered by this reciprocity request.
