INTERIM INITIAL EXAMINATION REPORT NO. 50-407/UL-91-01 On December 9, 1991, the NRC administered initial examinations to employees of your facility who had applied for a license to operate your University of Utah Reactor Facility.- The evaluation was conducted in accordance with NUREG-1021,
" Operator Licensing Examiner Standards,'" Revision 6. At the conclusion of the ;
examination, the examination questions and preliminary findings were discussed ;
with those members of your_ staff identified in the enclosed report. '
Due to problems identified during the grading of the written examination, a management meeting was held to_ discuss the examination results and to review ;
your operator licensing trai.ning_ program. The participants, findings and '
proposed _ resolutions discussed during this meeting are identified in an ;
enclosed report (Enclosure 2),
As a result of this meeting it was our understanding that you intended to accomplish several short term and long term corrective actions, described in i Enclosure 2. If oun- understanding of any of these short and long term actions is incorrect, please contact me within seven days of receipt of this letter.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter and the enclosures will be placed in the Public Document Room. '
