Pequest for Additionni Information Concerning Violation 50-482/8613-02 Daar Mr. Johncon:

The purpose cf this letter ir to provide additional information to that uubmitted ~in Reference 1, concerning the corrective action tarren in response te Violation 482/8613-02.

The violation has been placed in the electrical and techenical maintenan:e required realind Progrsm, (Required Eending Number 86-36), to ensure maintenance personnel are cognizar_t, of the imperts.nce of following precedures and properly decacenting the required information.

This action will reduce the posalbility of future similar occurences.

Although maintenance p3rsonnel did not properly document the required ir. formation, KLME does not believe that this single occurrence warrants a violation.

As. stated in Referer.ce 1 the werk requast program requires a final rev.lew of werk.requeste to ensure that the appropriate information is includei on work requests pricr to sending them to the vault.

Due to the fact that thia was an isolated occurrence s.nd the program in place would have found thin omission, KO&3 feels that thin would better be cle.saifi3d as a deviation rather than e violation.
