Dear Mr. Mcdonald:
In order to update our previous Quality Assurance Program in accordance with 10 CFR 71.12(C).(3) dated November 22, 1985 (Document number 71-0560), BMY requests to be registered as a user of Tech / Ops Inc., Model 650 package, Identification number USA /9032/B(U), under terms of Certificate of Compliance Number 9033 issued to Tech / Ops Inc., Burlington, Massachusetts.
All previous information covered in BMY's Quality Assurance Program for Part 71 will remain the same with the exception of Mr. W. Hicks (Vice-President) shall be removed f rom the Organizational Chart.
