This refers to the inspection conducted by Messrs. R. L. Cilimberg, C. Hirayama and Ms. C. Abbate of this office on July 21-25, 1986, of your facility at Windsor, Connecticut and to the discussions of our findings with Mr. G. H. Chalder and members of your staff at the conclusion of the inspection.

The purpose of this inspection was to review fabrication and inspection activiLies and Quality Assurance (QA) program implementation at your Windsor facility.

Areas examined during the inspection included a review of moisture control, pellet fabrication and inspection, chemical analyses of uranium dioxide, tubing inspection, fuel rod manufacturing and bundle assembly, equipment calibration, and personnel training and qualification. Areas examined during the inspection and our findir.gs are discussed in the enclosed report. Within these areas, the inspection consisted of an examination of procedures and representative records, interviews with personnel, and observations by the inspectors.

During the inspection it was found that the implementation of your QA program failed to meet certain NRC requirements. Weaknesses were observed during the helium leak testing when an operator failed to follow procedures and the required approval was not obtained for ink changes in that procedure. The ~ specific findings and references to the pertinent requirements are identified in the enclosure to this letter.

Please provide us within 30 days from the date of this letter a written state-ment containing:

(1) a description of steps that have been or will be taken to correct these items; (2) a description of steps that have been or will be taken to prevent recurrence; and (3) the dates your corrective actions and preventive measures were or will be completed. Consideration may be given to extending your response time for good cause shown.
