Limerick Generating Station, Units 1 and 2 Preliminary Replacement Operator Program Qualification Statement

Dear Mr. Miller:

Enclosed is the prelimintry (i.e., unsigned) Personal Qualification Statement (NRC Form 398) and Certificate of Medical History (NRC Form 396) for the Reactor Operator (RO) license candidate identified below. The candidate failed the Category B portion of the operating examination given at the Limenck Generating Station during the week of March 15,1999. Details of the waiver request for the portions of the examination the candidate passed are contained on the attachment sheet for NRC Form 398.

A tentative re-examination date has been set for August 12,1999.
